Dear Mrs. ZHERLITSYNA,

We report on your teleconsultation of February 6th, 2023.

Diagnosis:

Metastatic leiomyosarcoma (hepatic, peritoneal) ED 08/2022

  • Status post first-line therapy with 4 cycles of Gemcitabine/Docetaxel/Bevacizumab 09/2022 – 12/2022

  • Status post Pazopanib therapy for 7 days, including development of a “posterior reversible encephalopathy syndrome” (PRES)

  • Currently planned start of therapy with Trabectedin

Course:

The patient herself is not present at the teleconsultation, the consultation takes place with the patient's brother. A detailed and risk-adapted discussion followed. All questions were answered with the help of the translator. The patient's medical history has a Karnofsky index of 60%. She did not tolerate the first-line combination chemotherapy well, which was stopped due to hepatic progression. During second-line therapy with Pazopanib, the patient developed PRES. Currently, she has largely recovered from it, at least cognitively.

Procedure:

  • Re-staging after 3 cycles of Trabectedin.

  • If the disease progresses again, therapy with Caelyx (pegylated liposomal Doxorubicin) can be considered.

  • There is currently uncertainty regarding the presence of cerebral metastases. Re-evaluation of the submitted records, done at home, may be required.

  • Repeated consultation in case of need or complaint can be held at any time.

Best regards

Prof. Dr. Florin-Andrei Taran
