7. Specialist conclusion (established diagnosis)

ICD-10 code: C16.9 — Malignant neoplasm of stomach, unspecified.

Diagnosis:

Leiomyosarcoma of the uterine body, T2NxM1, secondary involvement of the liver and peritoneum. Status after 4 cycles of chemotherapy per the Gemcitabine + Docetaxel + Avastin protocol with disease progression in the liver (per CT, MRI of 10/2022). C-KIT and MET mutations detected (ONCOMINE). Status after treatment with pazopanib (05–12/01/2023) with development of posterior reversible encephalopathy syndrome and positive dynamics on CT.

01/2023 — repeat liver biopsy. Pathohistology — metastasis of uterine leiomyosarcoma. Status after 3 cycles of chemotherapy per the Trabectedin protocol with negative radiological dynamics (PET/CT of 11/04/2023). Suspected synchronous gastric neoplasia (GIST?). Status after EUS-FNA biopsy of the stomach and core biopsy of the liver; histologically — gastrointestinal stromal tumour (GIST) of the stomach, GIST metastasis in the liver (CSD).

08/2023 — status after 4 months of imatinib treatment with a pronounced positive radiological (PET/CT) response.

03/2025 — status after 2 years of imatinib treatment with a positive clinical picture.

05/2025 — stable radiological dynamics on imatinib treatment.

07/2026 — return of mural (wall) components in the known liver lesions on imatinib therapy after 3.5 years.

09/2026 — according to the FoundationOne study, mutations in exons 11, 13 and 18 of the KIT gene were detected.

8. Recommendations

Switch to sunitinib therapy 50 mg, days 1–28, followed by a 2-week break.

Taking into account the results of the FoundationOne study — a possible treatment option is regorafenib 160 mg, days 1–21 of each 28-day cycle.

Oncologist review + comparative PET/CT, or contrast-enhanced CT of the chest, abdomen and pelvis, after 3–4 cycles.
